An employee was straightening a pack of lumber due to a piece of wood sticking out. Using a scrap piece of wood, he struck the piece of lumber and his left index fingertip was caught between the boards and partially amputated.

Two employees were repairing a steam leak on a pre-dryer on a 20-foot elevated surface. While welding, both employees fell approximately 20 feet when they both lost their footing on the platform. One employee fell on a peg that punctured his right thigh and was subsequently hospitalized. The second employee received a head laceration that required 9 stitches. The second employee was treated and released the same day.
