On October 20, 2021, an employee was rigging a steel part to a chain hoist. The part fell, causing the chain to become taught and pinch the employee's finger. The employee suffered a partial amputation to the finger.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

Spellman High Voltage Electronics Corporation - Valhalla

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

An employee was using a box cutter when it cut the tip of his left thumb resulting in a fingertip amputation.

While working at a customer's residence, an employee was cutting tape with a box cutter to wrap insulation for a piece of pipe. The box cutter struck a hardened object within the wall causing it to bounce back and puncture the employee's right eye. The employee was hospitalized.

An employee was using the pipe-and-wrench method to loosen a seized bolt on a roof truss. A technician from another company was on the other side of the truss doing the same action with the same method. The employee's socket slipped off the bolt and his left hand was crushed against the truss, resulting in injuries to the index and little fingers, requiring hospitalization, surgery, and amputation of the index finger.

An employee was standing at the rear of a machine and the machine's mesh tray fell down onto the rear of the machine, crushing his left hand against the read lift. The employee sustained fractures and a laceration to their left hand and was hospitalized.

McLanahan Corporation

The injured employee was assisting with tightening down 3/4" hardware during assembly of a 6'x20' triple deck screen. Another employee was using a 1/2" cordless impact wrench to tighten down the hardware. The injured employee was using a 1-1/8" open-end wrench that slipped when the bolt was being tighten down by the impact wrench. The injured employee's right ring fingertip was caught between the open-end wrench and the side sheet of the triple deck screen. The tuft of the distal phalanx was amputated.

J.H. Fletcher & Co.

An employee was welding baffle plates onto a 55-gallon hydraulic tank. A flash occurred as they began to tack weld a part. The employee suffered burns to most of their face and second-degree burns on each arm, resulting in hospitalization.

Caterpillar Inc.

On July 22, 2024, an employee was using a pressure washer to fill a mop bucket with water. The washer's hose split and soapy water was injected into the employee's right palm and index finger. The puncture wound had to be surgically cleaned.

An employee was walking to their workstation when they were struck by a reversing forklift. Their foot was crushed and required surgery.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
