An employee was diagnosing the cause of a rear light malfunction on a forklift when the reverse gear engaged. The forklift moved backward and contacted the employee causing a metatarsal fracture in their left foot and injury to their lower right leg.

An employee was collecting shopping carts in the parking lot when he was struck by a customer's truck as it was backing out of a parking spot. The truck pinned the employee's left leg against the cart mule they were using. The employee was hospitalized with a fractured leg and required surgery.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee took a picture of the seal and license plate on a delivery truck after loading. The truck then reversed back to the dock and pinned the injured employee against the dock. The employee sustained an abdominal fracture.

An employee was between a milling machine and an 18-wheeler belly dump when the 18-wheeler backed up and ran over his feet and legs. The employee was caught between the front of the milling machine and the truck. The employee sustained fractures to their hip, leg, and feet.

The injured employee was assisting with tightening down 3/4" hardware during assembly of a 6'x20' triple deck screen. Another employee was using a 1/2" cordless impact wrench to tighten down the hardware. The injured employee was using a 1-1/8" open-end wrench that slipped when the bolt was being tighten down by the impact wrench. The injured employee's right ring fingertip was caught between the open-end wrench and the side sheet of the triple deck screen. The tuft of the distal phalanx was amputated.

J.H. Fletcher & Co.

An employee was welding baffle plates onto a 55-gallon hydraulic tank. A flash occurred as they began to tack weld a part. The employee suffered burns to most of their face and second-degree burns on each arm, resulting in hospitalization.

An employee was cutting a piece of rubber with a utility knife. The knife slipped and partially amputated the employee's right index fingertip.

On July 22, 2024, an employee was using a pressure washer to fill a mop bucket with water. The washer's hose split and soapy water was injected into the employee's right palm and index finger. The puncture wound had to be surgically cleaned.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
