An employee was in a locker room, leaning back against a locker while changing out of flip-flops into slip-resistant shoes. She fell backward, suffered a hip fracture, and was hospitalized.

The employee was replacing a cable in an ice storage bin. It came loose and pinched his hand between the wall of the storage bin and the cable, resulting in an amputation of their right little fingertip.
An employee was in the battery room changing the battery on a forklift. During this process, the battery started to roll back out of the forklift battery compartment after it was replaced with a fully charged battery. The battery fell and pinched the employee's finger between the top of it and the bottom of the battery compartment. The pressure crushed the employee's finger and resulted in a fingertip amputation.
