An employee was unloading the trailer on a powered industrial truck (PIT). As the employee reached through the mast of the PIT, his leg contacted the level control. The mast lowered and crushed the employee's right arm, resulting in injuries that required surgery to repair.

An employee was cleaning the gutters on a roof when he fell through a skylight and landed on the ground 20 feet below. The employee sustained fractures to their cheekbone and both wrists.

An employee was operating a forklift to pull a broken forklift out of the trailer when the broken forklift got caught on a dock plate. The employee exited the forklift used to tow the broken forklift to check on the stuck forklift and the mast of the broken forklift rose up catching the employee between the two forklifts.
