An employee was lowering himself to ground level from a drill rig platform when his left middle fingertip was pinched between the jaw of a hydraulic wrench and a section of the drill rig as the wrench was opened, resulting in an amputation of the fingertip.

An employee and his crew were performing drilling operations and just made a drill casing connection. While the drill operator was lowering the casing hammer and hose down to the casing, the employee was standing beside them guiding the hydraulic hoses with his left hand. When the operator lowered the casing hammer, the employee's right middle finger was contacted by the drill pin rod inside the casing rail, resulting in a compound fracture.
An employee was working to build an electrical substation. He was cleaning out the inside of a fixed section of a PVC pipe on the end that was furthest from an excavator.
