An employee was unloading an air conditioner unit from a pickup truck when the employee dropped the unit and fell backwards on the curb. The employee experienced swelling of the spinal area and was hospitalized with a lower back contusion.

On the morning of July 24, 2025, an employee was conducting synthesis of a highly toxic experimental chemical (nerve agent) in a chemical fume hood. The operation was completed at approximately 11:30 AM. The employee was exposed to the chemical and began experiencing symptoms of illness/poisoning around 1:30 PM and was hospitalized.

On March 5, 2025, a nurse was caring for patients in the emergency department when she was exposed to norovirus and rhinovirus. The employee tested positive for norovirus and rhinovirus and was hospitalized.

An employee was removing a carbon dioxide line when red liquid was found on the ground. The employee reported a "red spot" on their left wrist, inside elbow, and chest after washing down the spill. The employee experiencing escalation of symptoms, including a rash/burn on their arms and respiratory symptoms after the shift. The employee was hospitalized with chemical pneumonitis, chemical dermatitis, and chemical asthma exacerbation.

Employees were pulling a chemical sample from a device (sample port). The sample wouldn't come out because there were two samples in the port. The employees then turned off and deenergized the machine to 0 pounds per square inch. They unscrewed the top of the machine to retrieve the stuck samples when a small amount of pressure caused liquid to squirt out and onto one of the employee's face shields as the lid was removed. The employee was hospitalized due to chemical exposure.

A nurse suffered an allergic reaction while working on a hospital floor helping patients. The nurse went into anaphylaxis and was hospitalized.

An employee was walking into work when they tripped on a rock and fell forward to the ground. The employee sustained a fractured right patella and required surgery.

An employee was helping to restrain a patient when his right ankle rolled, resulting in a fracture.

An employee was helping to move a patient in the endoscopy room. After helping the patient into the bed, she went to stand back up and struck the back of her head on the C-arm table attached to the hospital bed. The employee sustained a concussion.

An employee was taking an x-ray using a portable x-ray machine. The employee was adjusting a cartridge under a patient and felt abdominal pain. The employee sustained an incarcerated hernia.

An employee slipped while cleaning up spilled eggs and landed on their left knee. The knee was broken.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
