An employee was placing a block of beef livers on the table of a band saw to be cut when the table slide moved and the block pushed two fingers on his left hand into the blade. The employee sustained amputations of his middle finger just above the distal knuckle and his ring finger at the base of the fingernail.

An employee was making adjustments to and cleaning product from pelletizer processing equipment when their right hand was caught in the rotary cutter resulting in lacerations and amputation of the little finger.
