Final narrative
A contract employee at the Texas Concrete location was loading (hooking and unhooking) concrete pile onto a flatbed truck. The product was securely positioned when the dunnage shifted/broke causing repositioning to be necessary. A crane operator adjusted the product and the injured employee's right index finger was caught between the hook and the lifting loop of the product, resulting in a fingertip amputation.

An employee was applying initial stress on a concrete cable using a jack when either the cable snapped or the chuck that holds the cable in the jack failed, creating a pinch point. The employee's left thumb was caught in the pinch point, resulting in a surgical amputation to the thumb.
