An employee was walking under an outrigger as an 8-foot aluminum beam was being lifted when the beam fell and struck the employee in the head. The employee sustained blunt force trauma to the a head and a laceration. The employee was hospitalized.

On December 10, 2016, at 9:30 a.m., an employee was removing shoring equipment when his right thumb tip was pinched and amputated between shoring and an I-beam.
An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
