An employee was removing cardboard that was caught in the top of a cage while standing on the outside of the powered industrial truck (PIT). The employee's left hand was crushed between the PIT fork and bar.

An employee was pulling a bulk-bag of peanuts weighing approximately 1,700 pounds using a forklift when another bulk-bag destabilized and fell on the cage of the forklift. The employee jumped from the forklift and when the bulk-bag broke open it rolled and landed on him, pushing his face to the floor. The employee sustained fractures to his skull, nose, maxilla, and orbital bone, as well as a facial laceration.

An employee was cleaning a line and removing buildup from the mixer exit point when his right middle finger came in contact with the mixer blade, resulting in partial amputation of the fingertip without bone loss.

An employee was walking when he slipped on water and chemicals on the floor and sustained chemical burns to his leg, arm, and back.

An employee was using a pallet jack when his right foot was crushed against a pole.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was operating a stand-up forklift. The forklift drove into a rack in a warehouse, and the employee's right hand was caught between the rack and an upright supporting the forklift's overhead guard. The employee suffered a fracture and laceration to the hand, just below the little finger.

An employee was pulling a rubber conveyor belt around the end of a line to connect it to the mechanical portion underneath. The employee's right hand was pulled into the gears under the belt, causing an amputation to the right thumb.

An employee was cleaning a label cutter drum with a rag. His left index finger was caught in the cutter resulting in amputation of the fingertip.

An employee was mopping a floor. When he went to move a portable spice spreader, it fell on him and lacerated an area behind his right knee. He was hospitalized, requiring surgery.

An employee was hanging a tote bag on a forklift tine when their upper arm became caught between the tote bag and the tine. The employee had the soft tissue of the upper arm crushed.

An employee was cleaning when peanut dust hit his face causing him to fall 3 feet from a ladder onto a metal object. The employee sustained a partial collapse of his right lung.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
