An employee was power-washing the area around a filter. The filter medium was pressurized and released; hot water and steam struck the employee, who suffered burns to the left arm (from the wrist to just above the elbow) and the back of the neck.

An employee was operating a tractor when it flipped and ejected him. Its back tire then rolled over him, and he suffered a collapsed lung, broken ribs on the right side, and lacerations to the left hand and right arm.

An employee had been operating a forklift. While releasing a dumper trash hopper into a dumpster, the employee's finger became trapped in the base of the trash hopper. The left index fingertip was amputated.

An employee pinched two fingers between the cable and pulley of a portable belt conveyor resulting in an amputation injury. The motor was not running at the time of the incident.

As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was conducting routine maintenance on a mixer. New lid pins had been installed, and the employee was rotating by hand to ensure they had been installed correctly. The employee's left middle finger was caught between the lid and top of a pin. The momentum from the mixer continued, causing a crushing injury to the finger. The employee underwent a medical amputation from the top knuckle to the tip of the finger.

An employee was pulling a rubber conveyor belt around the end of a line to connect it to the mechanical portion underneath. The employee's right hand was pulled into the gears under the belt, causing an amputation to the right thumb.

An employee was cleaning a label cutter drum with a rag. His left index finger was caught in the cutter resulting in amputation of the fingertip.
