When the pole came out of the ground, it swung up and struck the injured employee on the left side above his waist and the right side of his head. He was knocked to the ground and his left calf was scraped. The injured employee was hospitalized with a fractured rib and lacerations. He required surgery.

Employees were working to block a client from eloping. The client and employees were running down the hall when the client pushed the injured employee, causing her to fall and slide into a steel doorframe. The employee sustained an injury to her left arm/shoulder.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below.
