An employee was storing away a hospital bed in a warehouse when the bed collapsed and his left little finger got caught in the metal frame, resulting in a laceration to the fingertip that required hospitalization and a surgical amputation.

An employee was walking when they slipped on ice and fell to the ground in a parking lot. The employee sustained a fractured rib and a lung contusion.

A person donated plasma. Before leaving, their pepper spray accidentally discharged in the office and caused a chemical reaction. An employee was performing phlebotomy and sustained a seizure.

An employee slipped on a wet floor and fell. He was hospitalized with a back injury.

An employee stepped outside because she was feeling hot. She proceeded to become disoriented and fell onto the cement loading dock. The employee sustained a head contusion.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee knelt on their left knee to draw blood from the hand of a patient. She suffered a back injury involving a bulging disk and was hospitalized.

An employee ate an uncooked, company-provided chicken meal during lunch, developed a food-borne illness, and was hospitalized.

On April 3, 2025, an employee was pushing a heavy cart when the cart passed over a hole in the concrete floor and tipped over. The employee sustained hip fractures from the cart.

An employee was approaching a client's lockbox to pick up specimens. He tripped over the curb and fell to the ground. The employee was hospitalized with a broken left hip that required surgery.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
