An employee was doing roofing work when they became ill and experienced body cramps, resulting in hospitalization for heat stress.

An employee was using a corded high-torque electric drill to mix waterproofing materials. As he tried to remove the mixer from the bucket, the mixing paddle shaft caught the glove on his left hand. The glove was wrapped around the shaft as the drill spun, resulting in the amputation of his left index finger.

Two employees were needed to hold the cable while the third employee drilled the mounting locations. When the third employee activated the drill, the cables twisted. They caught the right index finger of one of the other two employees, amputating his fingertip.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
