An employee was diagnosing a car that wouldn't start. They sprayed starter fluid into the engine intake and another employee started the car while the hood was up. The car backfired and fire came through an open engine valve, burning the employee's face.

An employee was taking a break outside his work vehicle at a convenience store when he was attacked by a customer with a pair of scissors. He called 911. When the operator asked which way his attacker had gone, he stepped away from his vehicle and the attacker hit him with his van. The employee suffered a head injury; multiple broken ribs; abrasions all over his body from being dragged by the van; and injuries to the right shoulder, right knee, right ankle, and lower back.

An employee was providing roadside assistance in a highway breakdown lane. A passing vehicle struck the employee's vehicle, and he suffered a spinal injury, a punctured lung, and internal bleeding. He was hospitalized.

An employee was working on a catalytic converter when he slipped and injured his leg. His leg swelled and the employee was hospitalized.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

The injured employee was assisting a Department of Homeland Security (DHS) enforcement operation. During an apprehension, officers returned fire at a subject and struck the employee with a bullet. The employee sustained a gunshot wound to their right hand.

An employee was working at the store when an irate customer from the drive thru came inside to yell and spit at the staff. When the customer went outside again, she pulled out a gun and shot an employee in the chest. The employee was hospitalized.

An employee walked into the back office and an altercation occurred with another employee. The injured employee was shot by a firearm, once in their side and twice in their arms. The employee's left leg was amputated below the knee and their left arm was fractured.

The injured employee had been working on the computer at the front desk in the showroom. An argument occurred with another employee and additional people got involved. Gunshots were fired. The employee was shot in the hip and was hospitalized.

An employee was changing the location of bolts that held a metal plate on the top of a car wash conveyor. His thumb became stuck between a part of the equipment and a roller on the conveyor. He sustained a fracture to the right thumb just above the base knuckle, resulting in an amputation.

An employee was walking on a carwash roller track when their right foot was caught inside the roller track equipment. The employee sustained torn ligaments and tendons, a partial degloving, and an arterial bleed in the foot.

On July 17, 2025, after someone reported a burning smell at the car wash exit near the washing machine, an employee was inspecting the area. When the employee touched the washing machine, he was electrocuted and hospitalized.

An employee came in early to use the air compressor to add air to one of his car's tires. While he was walking across the parking lot, the injured employee noticed a man sitting in the parking lot. The unidentified man rushed the injured employee and stabbed him three times in the back with a knife.

An employee was washing the tunnel equipment area using a sprayer and a chemical containing 5-10% hydrofluoric acid. The chemical seeped into his clothing and caused chemical burns to his abdomen and upper thighs. The employee was hospitalized.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
