On September 25, 2024, an employee was hospitalized after being shot in the leg by another employee following a verbal dispute at work.

An employee was diagnosing a car that wouldn't start. They sprayed starter fluid into the engine intake and another employee started the car while the hood was up. The car backfired and fire came through an open engine valve, burning the employee's face.

An employee was taking a break outside his work vehicle at a convenience store when he was attacked by a customer with a pair of scissors. He called 911. When the operator asked which way his attacker had gone, he stepped away from his vehicle and the attacker hit him with his van. The employee suffered a head injury; multiple broken ribs; abrasions all over his body from being dragged by the van; and injuries to the right shoulder, right knee, right ankle, and lower back.

An employee was providing roadside assistance in a highway breakdown lane. A passing vehicle struck the employee's vehicle, and he suffered a spinal injury, a punctured lung, and internal bleeding. He was hospitalized.

An employee was working from a platform ladder, spraying a tank with water to clean it. The employee slipped; as she tried to steady herself, her right middle finger was caught in a pinch point. The fingertip was amputated and she was hospitalized, requiring surgery.

An employee was washing dishes when she slipped and suffered a fractured ankle.

An employee was working on a baker scaffold when he slipped, causing his left arm to strike the railing. His left arm was punctured from a stacking pin on the scaffold frame.

An employee was working on a washer. He was standing behind the machine and walked through the hydraulic absorbent pad. As he walked around the machine he slipped and put his hand down to brace himself. The employee contacted the machine's belt and pulley, resulting in a hand injury. The machine was not guarded at the time of the incident.

An employee was stepping out of his work van when he lost his footing and sustained a quadricep tendon rupture.

An employee was climbing a fixed ladder to gain access to the roof for an inspection. The bolts holding the ladder to the wall broke, resulting in the ladder and the employee falling approximately 14 feet to the ground. The employee sustained a concussion and a brain bleed as well as fractured ribs and was hospitalized.

An employee was walking down the office hallway when she stumbled and fell to the floor, causing a bleed from her femoral artery and swelling in the left leg.

An employee completed an inspection of an insured property's roof for hail damage. As he was getting on the ladder to descend from the roof, he fell approximately 10-12 feet and landed on his back on the concrete ground. The employee sustained 10 fractured ribs (4-5 displaced) and 5 transverse process fractures. The employee was hospitalized.

An employee was working outside inspecting vehicles at a body shop when he became ill due to heat. The employee sustained dehydration, heat exhaustion, and a muscle injury.

An employee was inspecting wind and water damage in a residential attic when they stepped onto drywall between two trusses and fell through the drywall. They fell approximately 10 feet to the concrete ground below and sustained a fractured right tibia.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
