An employee was riding on the side of an all-terrain forklift when they fell into the space between the gas tank and the right rear tire. The employee was run over by the the right rear tire, resulting in fractures to their neck, pelvis, and right leg.

Employees were moving a paving machine. The machine had been pulled forward, then put in reverse to back up a hill, when it revved up and went out of control. It had moved about 40 feet when one employee jumped out of it and was dragged under it. A second employee rode about another 40 feet, then jumped out to avoid a steep embankment. The first employee suffered a broken pelvis, and the second employee suffered a broken lower leg. Both were hospitalized.

An employee was operating a lawn mower. He fell off while making a turn and the mower ran over his right foot. The employee sustained a laceration on his right ankle.

An employee was driving a forklift. He fell off the machine during a turn, and one of its wheels broke his left foot. He was hospitalized.

An employee was traveling on a walker rider as it headed for the outer wall of the warehouse office. He jumped off the front of the rider which caused his legs to be caught between the base of the rider and the yellow safety rail. The employee sustained a fractured right fibula.

An employee was walking in a corridor when she tripped and fell on the floor. The employee was hospitalized with fractures to her left shoulder and left ribs, and she sustained an injury to her left elbow.

An employee was inspecting behind a cooker when he tripped on a gas line. He fell, suffering a broken hip.

On February 5, 2025, at around 12:00 PM, an employee was rolling a motor (60 lbs.) up into a bucket for disposal when the motor rolled over his left index fingertip. The employee sustained an amputation of the fingertip.

An employee was providing support for hurricane relief. He was sawing a tree when one of the tree limbs fell down and struck him. The employee sustained a severe laceration on the forehead and an open fracture of his frontal sinus/skull. The employee was hospitalized.

An employee was positioned at the withdrawal point of a nurse tank containing anhydrous ammonia. The employee was positioning a hand wrench at the top of the nurse tank when the wrench slipped and bumped the valve release. The anhydrous ammonia vapor released from the tank, resulting in an inhalation injury.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
