An employee was inflating a farm tire and wheel assembly in a commercial tire restraint cage. The assembly failed and the cage absorbed the energy from the release of compressed air from within the cage, and the cage contacted the employee's arm. The employee was hospitalized with a fractured arm.

On 7/29/2024, an employee was utilizing a hand dolly cart to move a stack of automotive tires. When he used the foot pedal to start moving the cart, he felt a pain in his knee/leg area. The employee sustained a torn quadriceps tendon.

An employee was performing rear brake shoe service on a truck. The employee was using a prybar to remove a spring when the spring slipped from the S-cam and amputated his left index fingertip.

An employee was cleaning a machine and bleeding its lines. After the gauge read 0, the employee removed the metal lid covering a filter. The lid popped off and struck the employee in the face, causing a broken nose, a broken jaw, and lacerations.

On 6/11/2025, a lineman was removing a dead tree off of a powerline that had fallen due to thunderstorms. The employee cut the top of the tree to lessen the weight of the tree. He then hooked a jib line to the tree to lift it off the power line and place it on the ground. As he was moving his bucket into position, the tree shifted and fell, and the powerline released its tension. This caused a whipping effect and either the power line or the jib rope struck the employee's jaw. The employee was hospitalized with two fractures to his lower jaw.

An employee was replacing a tire on a wheelbarrow when the tire exploded. The employee sustained fractures to his right hand and an abrasion to the right side of his face. The employee was hospitalized and required surgery on his right hand.

An employee was guiding a customer's automobile onto the alignment rack when the vehicle drove over the rack and pinned the employee against the alignment computer. The employee sustained a bruised left hand and a fractured left fibula and was hospitalized.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured.
