On October 30, 2024, an employee walked into a patient's room. The floor was wet from having been mopped. The employee slipped and fell on the wet floor, sustaining a dislocated left knee.

An employee was walking out of work carrying a crockpot in the rain. New mulch/mud had washed onto the sidewalk. The employee slipped on the mulch/mud and fell sustaining a right knee fracture requiring hospitalization.
At about 11:30 a.m. on March 14, 2022, an employee was preparing to move a patient to a new location. The employee tripped over cords or tubes leading to the dialysis machine being used to treat the patient, fell to the floor, and suffered a broken right hip. The employee was hospitalized.
