An employee was descending a vertical ladder on a container ship when he fell 12 feet to the platform below. The employee sustained fractures to ribs on his left side, his right knee, and his right foot, as well as lacerations and scrapes to both knees.

An employee was using a yard truck to transport an empty export container back to the ship to be loaded. While driving through the container yard, the yard truck went off the road and struck the concrete base of a light post. The employee sustained a laceration to his lip and a contusion and closed fracture to his sternum.
A crane was setting a shipping container in place 40 feet above a deck. A twist-lock (weighing 11.5 lbs.) fell off the container and struck an employee standing on the deck, hitting the top of his hard hat. He suffered a skull fracture and was hospitalized.
A truck driver was waiting at a freight terminal to receive an intermodal shipping container coming off a vessel. The crane lifting the container failed and the container fell several feet into the back of the flatbed truck. The driver was jostled in the cab, resulting in back pain and loss of feeling in his toes.

A dogbone link became stuck in the slot and needed to slide down approximately 1 foot to be seated. The employee went to lift the dogbone link up and adjust it when the link became free. A burr on the flange of the link snagged the employee's right glove. The link slid into a seated position and the employee's right middle finger was caught between the top surface of the barge and the top flange of the dogbone link. The finger was partially amputated at the midsection of the distal phalanx.
While preparing to install a new spool piece for a vapor and spent sulfuric acid line, an employee was working to ensure there was no residual product or pressure in the line before removing the 6-inch hose. The valve was cracked, and some spent acid still in the line sprayed out. The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.
An employee was securing a power cord to a refrigerated truck container. A truck started backing up to connect with the container and the employee was pinned between the tires of the container and the bolster of the truck, resulting in a fractured leg.
