An employee was climbing an extension ladder to access supplies. They were approximately eight feet off the ground when the ladder slid out from under them. The employee fell and struck the concrete floor, resulting in contusions to their head and torso, as well as head trauma.
An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight.
