An employee was turning an electric end-rider pallet jack from one aisle to another when they stopped to reposition the pallet jack. They backed up and their left ankle became pinned between the pallet jack and the racking, lacerating their ankle and fracturing the left fibula.

An employee was operating a pallet jack when their right leg became pinned between the pallet jack and storage racking. The employee sustained a fractured lower right leg and required surgery.

An employee was operating a golf cart with a package in the front seat. The package became wedged on the gas pedal as the employee drove up a ramp, causing the cart to go out of control and strike some racking inside a distribution center. The employee suffered three fractures in the pelvis area.

An employee was was exiting a car they were detailing when a malfunction caused it to move. The employee was pinned between the car door and a pole, suffering a lower left leg fracture and crushing injuries.

An employee was operating a tug that had been re-tagged due to faulty brakes. As the employee approached the designated parking area, he slowed the tug using the clutch and then exited the tug. He went to stop the tug from colliding with a concrete wall when his right ankle became caught between the tug and a parking stall barrier. He sustained a fractured ankle and was hospitalized.

An employee was on a tugger. He was backing up to pick up a stack of pallets. While aligning the tugger with the pallets, his left leg was caught and broken between the tugger and a storage rack.

An employee was operating an electric pallet jack. While standing beside the equipment, the pallet jack lurched forward, pinning the employee's leg against racking. The employee was hospitalized with a fractured right ankle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

An employee was operating an electric pallet jack when it pinned his right ankle against some racking. The ankle was broken and he was hospitalized.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
