An employee was putting out road barrels for traffic control and standing on the lift gate of a traffic truck with a driver in the driver's seat. The truck was struck from behind by a tractor trailer and the employee was thrown to the ground. The driver of the truck sustained injuries and was not hospitalized. The employee was hospitalized with a laceration to the head and a concussion, as well as a fractured femur that required surgery.

An employee tripped on the arm of a machine and fell into hot asphalt. He sustained burns and was hospitalized.

Jordan Excavation

An employee was operating a street sweeper following the paving crew in an active work zone. The street sweeper was struck from behind by an empty logging truck and the employee was thrown from the sweeper into the wood line. The employee was hospitalized for a skull fracture, two small brain bleeds, a laceration requiring 18 staples, an open fracture of his clavicle, a dislocated shoulder, and multiple scrapes and bruises.

Cashless Tolling Constructors

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

Anderson Columbia Co., Inc.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

HB Fleming, Inc.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

GERRITY INDUSTRIES

Two employees were working to change a tire on a mobile slasher saw. The slasher weighs around 2,000 pounds and is towed. Employee 2 was lifting the saw while the injured employee was placing a block of wood under the frame for support. The slasher then lowered onto the block and the injured employee's left thumb was crushed between the slasher frame and the wood block, leading to an amputation at the first knuckle.

An employee was using a metal shear when it amputated the tip of his right index finger.

Cives Steel Company New England

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

Longroad Energy

An employee was standing on a step stool, removing the nuts and bolts from the frame of a solar panel that was being replaced. The employee's cheek made contact with a connector with damaged insulation. The employee was shocked, briefly lost consciousness, and fell to the ground, suffering an injury to the left shoulder.
