An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.

An employee was opening the rear entrance door of the facility. Wind closed the door on his left hand, causing the little fingertip to be amputated.

An employee was repositioning a limiter inside an assembly fixture. The employee's fingertip was pinched between the fixture and the limiter, causing a partial fingertip amputation.

An employee was aligning bar stock in the collet of a CNC machine and troubleshooting. As the employee was making an adjustment in the collet cage, the machine was activated, causing the collet to move forward and pinch the employee's hand. A finger and the thumb on the hand were medically amputated.

On April 14, 2022, at 11:40 PM, an employee was unwrapping cure tape from a completed hose after vulcanization. The cure tape pulled on round tubing and degloved the employee's right middle finger resulting in distal amputation of the finger.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

Two employees were removing a service lift platform from the back of a radar system. On June 27, 2025, an employee was walking out of the hangar when he tripped and fell on the concrete ramp. He sustained lacerations to his right elbow and right hand, and was hospitalized with a fractured right hip.

An employee was loading a 150-pound block of aluminum into a CNC machine when their right hand was struck by the block. The employee sustained a fracture and severed tendon to the index finger, as well as a laceration to the middle finger that required stitches.

An employee picked up material out of a cardboard box that was on the floor. As he was carrying the material, he stepped backward and tripped over the cardboard box, causing him to fall and suffer a compound fracture of his leg.

An employee was on a pier mooring a small, rigid inflatable boat. Their fingers were caught between the boat recovery line and the pier piling when the recovery line came under significant strain due to rough seas, resulting in a severe laceration to the left index finger and medical amputation of the left middle finger.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
