An employee was delivering packages to a house. Four dogs got out of the house and attacked the employee, who suffered bites to the head, body and arm.

An employee was scanning in product and went to step across the tongue of the trailer. The employee tripped over the tongue and fell to the ground fracturing their shin in multiple places which required surgery.

An employee was opening the doors to a trailer while making a delivery when the wind caught the door and their hand was caught in the door. The employee sustained a displaced fracture of the distal phalanx of their left index finger.

After clearing a jam on a secured conveyor belt, an employee walked back to step down, lost their footing, and fell onto the ground outside a bay door. The employee was hospitalized with a fracture to their left arm/elbow.

An employee was performing deliveries when he felt light-headed and experienced cramps. The employee sustained a heat-related illness.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
