Final narrative
An employee was turning on a machine and searching for the work order to begin operations. To access the board and items for the work orders, the employee lifted open the roller conveyor gate. As he released one side of the gate by pulling the latch, the weight increased on the other side where he had to push. This resulted in the gate snapping back and catching his left index finger, leading to partial amputation.

An employee was moving an air-supported wall structure when the sweeper got stuck. While lifting it up, it swung back into place and amputated his left middle fingertip.
