An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

A truck driver was loading granite slabs onto a truck bed when he fell through a gap between the loading dock edge and the truck bed and landed on the ground in the bay below. The driver was hospitalized with a broken tibia and fibula, a dislocated shoulder, and contusions.

An employee was moving a granite slab with a gantry crane. When the slab was put on the ground, the crane's clamp hit another piece of granite and released the slab, which struck the employee. The employee suffered a punctured lung and broken ribs.

An employee climbed onto a collector conveyor belt to pry a large rock out from under a feeder grate. The plant was started and the employee was pulled down onto the conveyor under another feeder conveyor. He fell clear after about four seconds of the belt moving under him, having suffered several broken ribs and an internal injury.

An employee was scraping loose paint on a railcar door. The employee's hand was caught in the door when it closed unexpectedly. The employee sustained a fractured right wrist.

An employee was holding a slab of quartz. He tried to separate two slabs when the momentum and weight fell back on him. He then tried to push them back and step out of the way of the falling quartz, but he fell and hit his head. He broke his leg and required head stitches.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
