Federal OSHA recorded a severe workplace injury at Brightspeed of Northwest Arkansas, LLC, Corner of 8th and Dryer Streets, MOUNTAIN HOME, ARKANSAS 72653 on — Fractures , affecting the Head and trunk.

Employees were inspecting a broken telephone pole to remove it and attached the cable from the roadway. They removed the broken pole from the cable/strand so they could raise the cable/strand up and attach to a tree to gain clearance until it could be repaired. The injured employee cut the top (above the cable/strand) of the pole off and it fell to the ground in the roadway. They then moved that piece out of the roadway. This left a 6-foot piece still attached to the strand (below the strand) at about a 45-degree angle to the ground. When the remaining 6-foot piece was cut from the cable/strand, it broke and swung toward the injured employee, knocking him to the ground. The employee sustained a fractured skull and a fractured L5 vertebra.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

C. Miller Drilling Inc

A service crew was pulling a pump and motor from a well. After breaking a joint of pipe loose from another joint, an employee grabbed the chain tongs to prevent the pipe from spinning in the well, so the crew could continue taking it apart. The bolt that attaches the chain to the handle of the chain tongs broke, causing the chain to swing around and strike the employee's lower right leg. The chain severely punctured his leg and fractured his tibia.

Rolling Frito-Lay Sales, LP

An employee was using a handheld grinder to cut a bolt. The grinding wheel broke apart and a fragment struck the employee s right hand, causing a laceration to the palm near the little finger.

Blackburn's Fabrication, Inc.

An employee was operating a brake press when the top die broke, causing the bottom die to roll out and onto the employee's legs. Both of the employee's legs were crushed below the knee, and both legs were partially amputated. The employee's left leg was also fractured above the knee.

Textron Aviation Inc. (Orlando Service Center)

A pressurized aircraft nose landing gear strut assembly was being disassembled. The gland nut dislodged under pressure and made contact with an employee's left hand, breaking it.

Exacore Staffing LLC

Two employees were working by a switch gear. Upon opening the switchgear, employee 1 noticed some loose bolts. As employee 1 looked back, employee 2 reached into the switchgear and an arc flash occurred. They were both severely burned. Safety gear was worn at the time.

AT&T

An employee was walking on a concrete storm water drain canal when they tripped on the unlevel concrete surface and fell, breaking their left ankle. The employee was hospitalized for surgery.

Serverfarm ATL1 Atlanta

An employee was manually moving a dolly (loaded with 7 sheets of drywall) out of a workspace when the dolly tipped over and landed on his right ankle. The employee was hospitalized with a fractured ankle and required surgery.

Comcast

An employee was on a 28-foot extension ladder troubleshooting equipment at a residential property. While on the ladder, the strand cable supporting the extension ladder failed, causing the ladder and the employee to fall the ground and into the roadway. The employee sustained facial injuries and a dislocated left elbow.

Atlas Power

While installing a server into a data rack, an employee fell 6 feet from a ladder to the floor. The employee sustained fractures to his left shoulder and arm, as well as a bruise and laceration to his forehead.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
