An employee began his shift by turning on a pallet stacking machine so that the hydraulics could warm up. The machine then ejected a stack of pallets. As the employee was walking around the outfeed conveyor to turn on additional machines, he was struck by the falling stack of 21 pallets. The employee suffered a neck injury that required surgery.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.
