An employee was cutting steel banding on a bundle of galvanized pipe stored overhead while standing on the raised forks of a side-loader forklift. When the last band was cut, the bundle of galvanized pipe broke free and struck the back of his head. The employee fell to the floor below and sustained a laceration and contusion to the head with a loss of consciousness.
