A maintenance employee working from a 10-foot extension ladder was putting clips on the shade cloth. The ladder shifted, and the employee and ladder fell. The employee hit the transport rail (rollers), broke five ribs, and lacerated his lung.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was on the tenth step of a three-legged, 14-foot pruning ladder to prune a holly tree. The front leg of the ladder entered a hole in the ground made by a rodent, causing the ladder to become unstable. The employee fell backward onto the ground, resulting in a fractured T12 vertebra.

An employee was riding in a nursery vehicle with another employee driving. The vehicle went off the road and struck a wooden utility pole. The employee suffered a broken left femur, damage to the left elbow, and chipped front teeth. He was hospitalized, requiring surgery.

An employee's fingertip was crushed between a hydraulic clamping device and a post, resulting in an amputation.

An employee was carrying signs to the front of a nursery when they tripped on a curb and fell, resulting in a twisted left ankle and fractured right shoulder.

An employee was removing plant flats from a pulley conveyor and was adjusting plants on the conveyor. The employee put their left hand under a flat to fix it, and their glove got caught on a pulley, resulting in amputation of the left little finger.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
