Needing to move the bowls, the employee moved the cart. The bowls tumbled off the cart and bounced off the floor. While the employee was trying to stop the bowls from rolling, his right ring fingertip was crushed between two of them. He suffered a contusion and amputation to the fingertip.

An employee was in an aisle when their left foot was struck by a pallet jack. The employee was hospitalized with a fractured foot.

An employee was exiting the dispatch office when they slipped and fell while walking down the stairs, resulting in a concussion and a cut on their head.

The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was moving branches and other material when they were struck in the leg by the track of a skid-steer loader, resulting in an ankle and lower leg injury.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). The employee suffered bruising and swelling on her hips, lower back, knees, and left side; a puncture wound to her left thigh from a machine screw; a crushed right hand with numbness and tingling; numbness to the left big toe; and a right wrist sprain.

Employee 1 was investigating a gas odor. While employee 1 was checking the equipment, a gas explosion occurred, resulting in a fire that burned the employee's face. Employee 2 entered the area and a second explosion occurred, causing burns to their face and arms. Employee 2 was hospitalized.

An employee tripped while walking from one store register station to another. Her forehead struck a register, and she suffered fractures to two vertebrae in her neck. She was hospitalized.

An employee was pulling a display shipper using the provided strap. The strap broke and the employee fell to the floor, suffering a broken wrist.

The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
