An employee tripped and tried to catch themself. Their left hand was caught in a chain and sprocket that drives the rollers for a furnace, and they suffered a partial amputation to the middle finger at the first joint.

On July 5, 2025, an employee was working to lubricate the chain on an autoclave machine in the coating factory. Their left index finger was caught in the machine resulting in surgical amputation at the first joint. The machine was guarded at the time, but was not locked out/tagged out.
An employee was transferring a PVB table from the PVB stretch machine when the middle fingertip on his left hand was compressed between the PVB table and the base of the table holding the PVB table. As a result, he sustained a left middle fingertip tissue amputation (no bone loss or removal of fingernail).
A sanitation employee entered the chemical room and slipped on the wet floor. When he slipped, he kicked over a bucket containing cleaning chemicals. The chemicals splashed onto the employee's clothing and burned his skin.
