An employee was at a customer's address replacing a car windshield when he started to feel lightheaded and dizzy. The employee sustained heat stroke.
An employee was operating a pallet jack to change a forklift battery. Upon reversing, the employee's right leg became caught between the pallet jack and a scrubber, resulting in a right tibia fracture that required hospitalization and surgery.
