An employee was tracking an infeed belt on a scrap hogger when their right arm was caught between the belt and a roller. The arm was broken and the employee's shoulder was dislocated. The employee was hospitalized.

An employee was working in the converting area installing a glue tote, and became ill from the heat. The employee sustained heat exhaustion.

An employee was repairing a chemical leak under an ultrasonic cleaning machine while lying on the ground. The employee had used water to identify the leak. The floor got wet and potentially activated the chemical, a solution used for cleaning anilox rolls. The employee sustained chemical burns to their right shoulder, right forearm, and right buttock.

An employee was putting a machine back in service from being locked and tagged out. When the employee restarted the machine, hot water and broken down woodchips began to flow from the drain valve. The employee sustained first-and second-degree burns to both lower legs.

An employee was changing a leaking valve in the bleach department. After the employee disconnected the airline, the acid in the line sprayed onto their chin and neck. The employee was hospitalized for first-degree chemical burns.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

A temporary employee turned off a planer machine, opened the housing, and began clearing a jam. The machine's spinning blade caught his right hand and degloved the index finger.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

On October 22, 2025, an employee was working to put a conveyor chain back on the track of a conveyor when their fingers were caught between the chain and the chain track, pulling their right hand into the sprocket. Four fingers were amputated.

An employee was cleaning the headers of the core machine using a hand drill with a brush attachment. His glove was caught on the brush shaft, resulting in amputation of the left middle fingertip.

An employee was operating a winder paper mill machine when their right index finger became caught in the machine. The finger was partially amputated at the first knuckle.

An employee was lubricating a chain and sprocket on a corrugator splicer. He lost his balance, and his right middle finger came into contact with the chain and sprocket. The fingertip was amputated at the nail bed.

An employee was entering a building and reached out to catch a closing door. The door closed on the employee's finger, resulting in the partial amputation of the fingertip and nailbed.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
