Final narrative
An employee was stabilizing a pipe to be cut. The weight of the pipe shifted as it was cut. The 12" pipe flange laned on their right foot, resulting in multiple toe fractures.
An employee was descending a 20-foot job-made wooden ladder that was wet. As he approached 5 feet, his hands slipped off the ladder and he fell to the ground. The employee sustained a fractured right femur that required hospitalization and surgery.
An employee was inside a mechanical room undergoing renovation. He went to open the exterior loading doors to provide light. When he pushed the doors open, his momentum carried him through the opening and he fell approximately 6.5 feet to the asphalt below. The employee sustained a fractured right hip.
An employee was installing a control box outside in the shade and started feeling lightheaded. The employee later passed out. He was hospitalized for dehydration.
