On November 9, 2024, an employee was lubricating a drive chain while it was moving, and his left thumb was caught by the chain. The employee was hospitalized with an injured thumb, which required surgery. The machine was not guarded at the time.

An employee was preparing to unload a truck at a loading dock. There was an exposed wire hanging from the ceiling of the loading dock, and when the employee opened the dock door, the door contacted the wire, causing the employee to suffer an electrical shock.

On 6/22/2022, an employee was placing a shirt on a heat transfer machine when the heat plate came down on her hands resulting in burns to both thumbs.

An employee attempted to pull down a delivery truck's roller door. The employee adjusted his foot position on the rear platform of the truck when he fell and landed on the parking lot, resulting in a fractured hip.

An employee was traveling down an aisle on a powered pallet jack and a co-worker (injured employee) was traveling closely behind on a reach truck. The employee on the pallet jack turned into an aisle and stopped. The co-worker on the reach truck also turned into the same aisle and struck the forks of the pallet jack, crushing their left foot and resulting in a fractured toe and an amputated toe.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee's left arm was caught in the fingers or jaws of the head stretcher machine, resulting in a laceration. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
