An employee was operating a forklift on the roadway near a building. While backing up, the rear tires left the pavement and sank into the soft ground causing the forklift to flip onto its side. The employee s right leg and foot were trapped underneath the forklift resulting in multiple broken metatarsals.

An employee was retrieving a can from an ammunition can sealer. The machine severely lacerated his right thumb, which required hospitalization and surgery.

An employee was working at a station that packs parachutes into canisters. The hydraulic ram on the parachute packing machine initiated out of sequence, catching the employee's glove. She was able to extract her hand from the glove, but not before the end of the ram entered the canister and amputated her right middle fingertip.

An employee slipped on the step of the tractor trailer delivery truck and struck his right knee on the step. He was hospitalized for surgery.

An employee was using an excavator to unload concrete traffic barriers. The excavator swung, tipped over an existing barrier, and fell into a ravine/wetland. The employee sustained a fractured left shin requiring hospitalization and surgery.

A.L. Helmcamp Inc.

Two employees were driving a water truck pulling a trailer on the sloped shoulder within the barricade of a work zone when the truck and the trailer rolled over. One employee sustained a broken femur and was hospitalized. The second employee sustained contusions to the lower back left side and left forearm and was not hospitalized.

Lyondell Basell

An employee was driving a utility vehicle and making rounds in the facility. While transitioning from the paved portion of the roadway to the uneven gravel portion, the vehicle overturned and the employee's left foot was caught between the vehicle and the ground. The employee sustained a dislocated left ankle.

Primoris Renewable Energy

An employee was driving a telehandler (forklift) on a rural road with drainage ditches on both sides of the road. A tube of grease rolled near the operating pedals of the telehandler and when the operator bent over to grab the tube, the telehandler veered off the road. The tires of the telehandler slid on wet grass and went into a ditch. When the telehandler got to the bottom of the ditch (approximately 15 feet below), it tipped over onto the cab's passenger side. The employee was wearing a seatbelt and struck the interior structure of the telehandler cab and sustained a fractured right clavicle requiring hospitalization.

A crane operator was transitioning a crane from a slope to a flat surface when the crane began tipping over. The operator jumped from the cab of the crane and landed on the bridge deck as the crane overturned on the bridge. The employee was hospitalized with an injury to the left arm.

Calvary Industries Inc

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.

Sharp International Services, LLC

An employee was removing a hose from a chemical tank after filling the tank with hexamethylenediamine (HMD). He checked the line to ensure that the hose was clear, then disconnected the hose at the connection. Residual chemical sprayed onto his face, causing a second-degree burn. He was hospitalized.

Borealis Compounds, Inc.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

An employee was opening a filter housing in the potassium chloride production area and got sprayed with a hot solution of potassium chloride brine. The employee sustained thermal burns to their right hand and legs.

Polymer Solutions Group, LLC

An employee was performing maintenance on a guillotine cutter machine. As the employee was placing a pin in the machine, his left middle fingertip was caught, resulting in a partial amputation to the finger. The machine was not locked out at the time.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
