An employee was on a catwalk performing maintenance on the deck of a ship. While preparing to descend a ladder, the employee's foot missed the lower rung and he fell sideways onto the deck. The employee sustained fractures to his arms and was hospitalized.

An employee was picking up trash and pieces of broken pallets on the dock. He was struck in the back by a forklift that was transporting two large pallets. The impact caused him to fall onto the ground, and then the forklift ran over his legs. The employee was taken to the hospital with fractures to both legs.

An employee was walking on a catwalk on a ship examining a container and fell approximately 7 feet through an open ladder way. The employee sustained fractured ribs and fractured a lumbar vertebra.

An employee had been operating fans with wind-power equipment. The employee fell backwards, off a ladder, approximately five feet to the lower surface. The employee sustained a lower back and tailbone injury.

An employee was struck by a forklift during offloading operations and sustained a broken lower leg and ankle and five broken ribs.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was unloading frameless mirrors from a trailer. When unstrapped, the mirrors fell over onto the employee, who suffered a skull fracture, a brain bleed and blood clot, a broken left ankle, and lacerations.

A temporary employee was rigging secondary steel when the steel joist fell to the ground and struck his lower left leg, resulting in fractures to the tibia and fibula.

Two maintenance employees were dismantling a machine part (steel cylindrical rollers) on the ground. After unbolting a portion, part of the assembly fell approximately 6 inches onto one employee's left little finger. The employee was hospitalized and about 3/4" of their little finger above the top joint was surgically amputated.

An employee was stacking a pallet when the pallet fell, injuring her left leg. She was hospitalized and had surgery on her left leg.

While preparing to install a new spool piece for a vapor and spent sulfuric acid line, an employee was working to ensure there was no residual product or pressure in the line before removing the 6-inch hose. The valve was cracked, and some spent acid still in the line sprayed out. The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

An employee was standing on the outside edge of a hopper barge while watching a skid steer push ore for an excavator (with a clamshell bucket) to unload. The excavator was on a work barge. The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.

An employee was inspecting a conveyor to see why it was not running. The employee entered the the conveyor motor area, tripped on a piece of angle iron welded to the floor, and fell. The employee sustained a fractured orbital bone.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
