Final narrative
An employee was loading a trailer that required slip sheets to be placed between the stacks of product. He went to get onto the push-pull lift to load more product when he tripped over extra slip sheets on the floor and fell against the shipping container wall. The employee sustained a dislocated left shoulder.

An employee was helping to clear a pallet jam in a pallet exchange machine when the pallet machine released and pinched his left middle fingertip, resulting in a fingertip amputation.
