The employee collected an asphalt ticket from the driver of an asphalt truck and was walking back to the paving operation when the asphalt truck backed over the employee. The employee suffered multiple leg fractures and lacerations and was hospitalized, undergoing a partial below-knee amputation.

They were 40 feet away from a pile-driving operation when a piece of the cast iron pile-driving hammer broke off and struck the employee's hand.
