They were 40 feet away from a pile-driving operation when a piece of the cast iron pile-driving hammer broke off and struck the employee's hand. The employee sustained a severed artery, laceration, and fractures in their hand, as well as contusions to a leg. The employee was hospitalized and required surgery.

The employee collected an asphalt ticket from the driver of an asphalt truck and was walking back to the paving operation when the asphalt truck backed over the employee.
