An employee was deburring a threaded shaft on a CNC lathe. The employee's sleeve was caught in the lathe. As he was dislodging the sleeve, both arms were caught in the lathe. His arms were wrapped around the threaded shaft, breaking both arms and removing flesh from the forearms.

An employee was setting up a press brake and changing from a short die to a long die. While he was holding the lower die partway into position, the press brake was activated via the foot pedal. The lower die then slid fully into position, taking his hand with it. The press brake moved to the setup position and crushed his left hand, breaking his middle finger and dislocating his index finger. He was hospitalized and had surgery.
