An employee was on the back of a box truck, working to maneuver a roto tiller away from the metal tool rack that hangs on the walls of the truck. His left ringer fingertip became pinned/stuck between the metal handle of the roto tiller and the metal tool rack, resulting in amputation of the fingertip.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
