An employee was walking at ground level toward a manhole, carrying a manhole removal device. The employee tripped and fell onto the manhole removal device, resulting in multiple fractured ribs and a punctured lung.

An employee was using a 71.3-pound jackhammer to break up a concrete pad. The jackhammer became stuck in the concrete. The employee tried to pull the jackhammer out of the concrete and sustained a hernia requiring hospitalization and surgery.

An employee was ascending a ladder to access wiring on top of a chiller unit when they fell to the ground. The employee sustained internal bleeding near their hip and a broken rib.

An employee was on an A-frame ladder changing lights when they fell from the third step, resulting in rib fractures.

An employee was walking down the stairs when she lost her footing and fell down the stairs. The employee scraped both hands and her left knee, and struck a door at the bottom of the staircase.

An employee was terminating cables in a junction box. A loose ground wire came into contact with the bushing, causing a flash that burned the right side of the employee's face and his right hand.

An employee was using a bucket truck hoist to raise secondary aerial wiring. The wire made contact with the primary wire, causing an arc flash. The employee suffered burns to both hands and was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee was installing a fuse in a machine cabinet. An arc flash burned the employee's left arm, and the employee was hospitalized.

An employee's hand was caught in a conveyor belt as they worked to clear a jam in a piece of equipment. The employee suffered a dislocation to the right elbow, as well as a broken ulna near the elbow, and was hospitalized.

An employee was changing AC filters while on a ladder. The employee stepped up to the second-to-last rung when they slipped and fell about 5 feet to the ground. The employee sustained fractures to their scapula and ribs, a collapsed lung, and a back injury.

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

On August 19, 2025, an employee became ill and dehydrated after working outdoors for eight hours, participating in training activities. He was hospitalized.

An employee was ascending a fixed ladder and struck his head on an overhead object that was approximately 20 feet above ground level. The employee sustained neck injuries and tingling in his fingers. The employee was hospitalized.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.

An employee was conducting a delivery of beverage products and began having cramps due to dehydration. The employee was hospitalized.
