An employee was moving a light plant. When he pushed on it, its dolly/jack broke. The front of the frame fell, caught his glove, and pulled his hand down with it. It landed on the asphalt, pinching his right ring finger and partially amputating the fingertip.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.
