An employee tripped and fell while walking and holding a steel pipe. The pipe fell on the employee's left forearm, resulting in a compound fracture.

On July 20, 2022, at 11:30 AM, the injured employee was preparing to set up a dump trailer in the wash bay so the water could drain out of the dump bed. A co-worker lifted the dump trailer at the corner so the injured employee could set in place 5-6-inch long 2-inch square safety blocks. As the employee placed a safety block on the trailer frame under the raised dump bed, it dropped off the forklift fork onto their right index finger. The finger was fractured and about half of the flesh from the backside of the finger was removed without bone loss.
An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.
The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.
