Final narrative
The injured employee was assisting a skid steer operator to move a stack of bundled railroad ties. The operator lifted the stack of bundled ties and the band securing the bundle broke, causing the ties to fall and strike the injured employee. The injured employee sustained a fractured hip and both ankles were fractured.

An employee was trying to clear a jam of wood debris (cut-offs) at a transition between a flat belt and a shaker-conveyor. When the employee grabbed a cut-off, it caught the belt and wedged the employee's left index finger between the belt and the shaker-conveyor surface. The finger was amputated. The machine was unguarded at the time of the incident.
An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.
An employee was pre-drilling holes in ties on which to set plates and spikes. As the employee pulled the hand drill from a guide hole, the drill caught his pant leg. The drill then went into the right side of his left foot, resulting in a laceration that required hospitalization.
A temporary employee was tacking end plates onto a railroad tie by hand. They reached over to the press that completes the attachment of the end plate after the rail tie crosses over a limit switch. The limit switch activated, resulting in a crush injury to the employee's left index and middle fingers that required medical amputation. The machine was guarded at the time.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.
An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.
An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.
An employee was using a telehandler to move pallets of pellets from a pod. The employee exited the telehandler to pick up a sacrificial pallet that is placed under the bottom layer of pallets. When the employee turned to pull the pallet away, the top two tiers of the pod fell on him. The employee sustained a fractured ankle, three fractured ribs, and fractured vertebrae.
An employee was preparing to replace a belt on a fan drive assembly. He went to manually stop the fan and was caught by the belt. His hand was pulled into the machine and his left little fingertip was amputated.
An employee was cutting remanufactured lumber with a chainsaw when a board fell and struck his left hand, causing his left hand to move off the chainsaw. While he was moving his hand back, the chainsaw kicked back and lacerated the top of his left hand. The employee was hospitalized.
