An employee was gathering shopping carts in a parking lot. He lost his footing and fell forward, knocking a shopping cart over and landing on it and the asphalt. He suffered a broken left hip and was hospitalized.

An employee was cutting pork ribs on a band saw. A section of the pork ribs became stuck on the band saw, resulting in amputation of their left index finger.
An employee was cleaning the floor in the meat department. He slipped on a drain grate that had been removed for cleaning, landed on the floor, and suffered an injury to the left hip and leg.
